Respiratory Medication
Med4Home® provides a variety of commercially available nebulizer medications. Our medications include: Albuterol Sulfate, Ipratropium/Albuterol, Ipratropium Bromide, Yupelri®, Budesonide, Perforomist® and Arformoterol.
Nebulizer Therapy
Nebulizer compressors currently available are the Pari Trek® S Portable Compressor Nebulizer and Salter AIRE Elite™ Compressor.
Medication Orders
Medicare billing prescriptions should include the beneficiary’s name or Medicare Beneficiary Identifier, order date, quantity to be dispensed, treating provider's name, National Provider Identifier, and signature. Orders can be faxed to 800-805-8365 or sent electronically through Surescripts or Parachute Health.
Medical Documentation
Medicare requires that documentation of medical necessity be supported in the prescribing practitioners’ records. We obtain the required chart notes to ensure we can support the medical necessity in order to supply your patient with the medication(s) that were prescribed. This documentation will also be available for Medicare if the patient’s shipment is audited.
